How to Complete a Scout CRB Form

Thank you for offering to help the Scout Group as a volunteer or parent helper.

This process is free for the Scout Association and we as a Group do not pay any administration fees. The CRB process can take between 2 to 4 months to be returned.

You will need to follow the instructions very carefully as certain sections on the form are not required for Scouting purposes.

The form needs to be completed in CAPITALS & BLACK ballpoint pen

If you make a mistake on the form please do not use Tippex or cross it out, instead please ask for a new CRB Form.

Section A – Applicants Details

Please complete all the relevant boxes including National Insurance Number

Postcodes will need to be without the space between

Section B – Details of Position

This section has already been completed for you by the Scout Association

Section C – Additional Personal Details

Please complete all the relevant boxes

To all females: Please make sure you fill in your previous Maiden or Married Name if Married

You must put the Town and County of your birthplace from your Birth Certificate

If writing a telephone number please DO NOT leave a space between the numbers

Section D – Previous Addresses

Please complete this section if you have moved within the last 5 years

Supplied in the form is a continuation sheet on which you can detail more than one address

Make sure the dates lead on to your current address

Once you have completed the form you will need to take this to your

childs Section Leader or Group Scout Leader - Neil,

with the following identification:

· Passport (must have at least 6 months left to run)

· Driving Licence

· A Utility Bill confirming your address

· Birth Certificate (not a duplicate)

We suggest that you bring these items and the form to a meeting night, and then the Leaders will be able to quickly look at your identification and write down the numbers into Section X which is for the Leaders to complete.
